Client action requested: All Payer Claims Database opt-in, opt-out

Twenty-three states have laws requiring third-party administrators, health insurers, and other entities to report health care eligibility and claims to the state for compilation in a database referred to as an “All Payer Claims Database” (APCD). The APCDs use the data to help improve health care quality, reduce costs, and promote transparency.

As a Cigna Healthcare client, you have the right to opt-in or opt-out of the APCD eligibility and claims reporting. To learn more about APCD and to exercise your right, please click here.

Once you have read the announcement and determined your plan will opt-in to the APCD, complete the appropriate form(s) below and return your signed form(s) to AllPayerClaimsDatabaseAPCD@evernorth.com.

  1. If any employees reside in or your policy originated in the state of Arkansas, California, Colorado, Connecticut, Delaware, Georgia, Indiana, Kansas, Maine, Maryland, Massachusetts, Minnesota, New Hampshire, New Mexico, New York, Oregon, Rhode Island, Texas, Utah, Vermont, Virginia, Washington, or West Virginia, please complete the All States Form.
  2. If any employees reside in or your policy originated in New Hampshire, please complete the New Hampshire Form in addition to the All States Form.
  3. If any employees reside in Utah, please complete the Utah Form in addition to the All States Form.
